IMPACT Connect partners, who participate as either members or affiliates, form one of the most comprehensive, community-wide systems to connect health and social care, and track outcomes in our nation. And our network continues to expand.
IMPACT Connect Members
Our members are committed to improving population health and wellbeing, enhancing system efficiency, and advancing health equity. Using a shared leadership model, members and community-level partners shape the direction of the community information exchange by serving on our Governance Board.
IMPACT Connect Affiliates
As part of IMPACT Connect, our affiliates are able to work together to better coordinate care and tackle social determinants of health. Combined, they represent broad and diverse sectors of need including housing and homelessness; healthcare; behavioral healthcare; legal; safety; food; neighborhood services; and home repair.
